VTR Forms
These are provided here for those who may need additional forms signed from posts that were undertaken prior to the introduction of the ePortfolio. For posts commenced from August 2007 all 'signing off' documentation is completed on the ePortfolio and you do not need these forms.

Completed VTRs should be sent to the deanery for endorsement.

DCH: Theory+ Practical 14th - 5th July 2010 (for Theory in September and Practical in November 2010).
